Because Adventure Doesn’t Wait

Take your health into your own hands.

Like you, our team is active and engaged in the community, from mountain biking to chasing kids around to gardening. We know what it’s like to be off our game due to injuries, pain, and health conditions. With our experience and evidence-based approach, we treat each patient individually and collaborate with you on your care. From years of practice, we’ve learned that the best way to get you back to doing what you love is to teach you about your body, give you the tools to take charge of your recovery, and empower you to achieve your goals.

Together, we’ll help you get back to your adventure – whatever that may be.

About Us

You Focused.

At Monument Physical Therapy, we are your adventure partners – here at every challenge and setback, every celebratory moment and step forward. We work in chorus with your body’s unique needs and provide evidence-based healing from start to finish. We treat the cause and not the symptom, allowing you to get back to doing what you love.

Get back to your adventure.

We’ve Got You Covered

e accept self-pay patients who do not have insurance or prefer to pay out of pocket.

We accept all insurance.

We are in-network with:

  • Holista (Aetna, GEHA, etc.)
  • Medicaid
  • Medicare
  • Provider Network of America (PNOA) (covers many Cigna, Humana, and UMR plans)
  • Rocky Mountain Health Plans
  • UnitedHealthcare

Please note that Trigger Point Dry Needling is not covered by most insurance and must be paid by the patient. Each treatment is $50.

We welcome patients with an out-of-network insurance provider as well. Many insurance providers offer out-of-network benefits that may cover a portion of the cost of treatment.

While we are in-network with the providers listed and will verify your benefits, all commercial insurance is plan specific, so the best way to determine your status and coverage is to contact your insurance. We cannot guarantee what each plan will cover, and the patient is responsible for what is not covered.

Please review our Notice of Patient Privacy Practices for information about how we protect your personal health information.
